Candace Nagata MSN, is our 2009 Case Manager of the Year !!!
Candace received multiple nominations from her peers as the 2009 Case Manager of the Year. She is currently employed as a Disease Care Manager for Florida Hospital Healthcare Systems. She is described as passionate about her role, a mentor for moticational interviewing, and a true example of what a nurse, case manager, and friend is all about. She is never too busy for a peer conference regarding a difficult case. She is a advocate for her patients to ensure better outcomes for them.
